Well done guys, I had a look at this thread last night and had a whole bunch of thoughts but you beat me to it.
is it a faulty cap, should be fine with a good cap in place?
Couple of things you may want to try.
must do, you have your heater wires coming through a drilled hole that still has sharp edges, unless you have a death wish you must sort all of these penetrations out.
Your first cathode cap and resistor is connected to a ground buss that runs past all circuit, it may be a bit quieter if you run a single wire directly back to the filter cap for this connection. i.e. don't share the ground wire for the first stage as it is the most sensitive to ground loops.
You have grounded the HT centre tap on one bolt and then grounded the 47uf cap on another, you have also grounded the input jack to the chassis, again it may be quieter if you ground the centre tap and cap at the same place and more importantly isolate the input jack from the chassis and run a ground wire to the first stage cathode ground, this again might just help avoid ground loops

Also meant to say you need to twist your speaker out jacks by 180 degrees, so that when the jack plug is inserted the weight of the cable keeps the tip pushed against the contact, small things but it is not by accident that MikeZ does it that way....
